For most of my career, patients came in after cataract surgery thrilled with their new vision. Colors brighter. Signs sharper. Faces clearer. That has not changed.
What has changed is what they say a few months later about driving at night.
The daytime is beautiful, they tell me. The night is worse than before the surgery.
Headlights explode into starbursts. Brake lights blur into a solid wall of red with no space between cars. Streetlights grow rings. A traffic signal seems to melt downward. Every bright point of light on the road bursts outward into rays that cover the lane and cross into the other one.

Before surgery, her natural lens could flex and focus. When a bright point of light hit her eye, the lens fine-tuned itself and brought that light down to a clean, sharp point. A headlight looked like a headlight. A brake light looked like a brake light.
The artificial lens now inside her eye is fixed. It cannot flex. It cannot fine-tune. When a bright point of light hits it, instead of focusing that light down to a clean point, it scatters it outward into rays and rings and starbursts.

Two things stacking on top of each other. Neither one caused by her, or by her doctor, or by her surgery.
Your natural lens could flex and focus a bright light down to a clean point. Your artificial lens cannot. So it scatters each point into rays instead. That is why every headlight, brake light, streetlight, and traffic signal bursts across the road at night.
But here is the part that matters most. The burst can only be as big as the light feeding it. Cut the harsh light before it reaches the eye, and the lens has far less to scatter.
The important thing to understand is what this means for what can actually help her.
